Our approach
The mission of Terrace Health is to provide personalized care with dignity, respect, and compassion. We will do this by committing to quality service focused on keeping patients active and independent in their own home.
That is why we built our services as a continuum rather than a menu. A patient who begins with a few hours of personal care each week can move into skilled home health after a hospital stay, and later into hospice, without leaving the team who already knows them.
Our services
Every Terrace Health patient is assigned a care coordinator who stays with them across all three.
Trained caregivers supporting the everyday tasks that keep independence intact — bathing, dressing, meals, medication reminders, and companionship.
Often paid through long-term care insurance, Veterans Affairs, Medicaid, or privately.
Medically necessary skilled care after a hospitalisation or injury: nursing, physical and occupational therapy, and speech therapy, delivered at home.
Typically covered by Medicare, most private insurers, Veterans Affairs, and Medicaid.
Comfort-focused care for patients facing a terminal illness, wrapping the whole family in clinical, emotional, and spiritual support.
Covered by Medicare for those over 65, Veterans Affairs, Medicaid, and most private insurance.
